Tim Doyle - Regional Manager - Detroit, MI
Tim currently serves as the Regional Manager of Form Tech’s Detroit office, and is directly involved in projects throughout Michigan, NW Ohio, and Northern Indiana. He also participates in the Major Projects Group within the company. Tim has many years of service with Form Tech, and is one of the most tenured people in the company today. He joined the company in 1981 in an outside sales role at the company’s original location in Detroit. After nearly 13 years, in 1993, he relocated to Pennsylvania and established the company’s operation in Pittsburgh, where he built the business for seven years prior to returning to Michigan in his current role. Tim’s 35+ years of experience, field know-how and countless successes have made him a trusted advisor to many contractors throughout the region. |
Jim Prager - Regional Manager - Pittsburgh, PA
Jim joined Form Tech in 2006 as a Regional Manager and is based in the company’s Pittsburgh facility. Jim is a proven and respected industry veteran and is highly competent in all formwork applications. In addition, he brings 30+ years of experience to the company’s Major Project Team. Prior to taking his management role at Form Tech, Jim worked for George L. Wilson and Waco Scaffolding & Equipment in various outside sales roles for 20+ years. Jim attended Slippery Rock State University and earned a Bachelor’s degree in Education in 1980. |
Gary Jividen - Director of Branch Operations
Gary joined Form Tech in 2001 and immediately established the company’s Charleston, WV location. Gary’s high level of formwork and concrete accessory product knowledge combined with his on-site field experience has made him a great resource for contractors in the region and also our operations and sales teams. In 2023 Gary stepped into the role of Director of Branch Operations to help bring his knowledge and expertise into maintaining quality standards across all of our locations. For the 13 years before joining Form Tech, Gary served as a Branch Manager for Wolford Contractors Supply and later as an Industrial Account Manager for Patent Construction Services, where he managed the field operations of large industrial scaffolding projects. |
Chester Chikosky - Design Department Manager
Chester joined Form Tech in 2008 as a Systems Designer, and in 2015 took on the role of Design Manager, where he is now responsible for directing Form Tech’s drafting/design personnel in the design, scheduling, and completion of all formwork layout drawings. In addition, Chester is directly involved with the continuous improvement of Form Tech’s CAD software and is directly involved in the design of the most complex customer formwork requirements. Chester attended the University of Akron and received a Bachelor’s degree in Construction Engineering in 2005. Prior to earning his degree, Chester worked for Waco Scaffolding & Equipment for five years as a Scaffolding & Shoring Erection Specialist, where he directed the field assembly crews, and then for another eight years as a Draftsman and Component Testing Specialist. |
Tim Fondelier - Director of Consumable Sales
Tim joined the Form Tech team in 2018, establishing a new position as Inventory Sales Manager. He leads the company’s effort to procure and supply concrete accessories to our customers, including expanding our relevant product offering in each of Form Tech’s branches. Tim is a wealth of knowledge and serves as our internal expert for employee training and job specifications. Tim’s extensive experience comes from over 33 years working with accessories for the construction industry, from understanding how products are utilized to selecting the best products for any situation. Prior to joining the Form Tech team, he spent 13+ years as Regional Sales Manager covering all business segments for accessory manufacturer Dayton Superior, and the previous 20+ years in various outside roles with dealers George L. Wilson and Waco Scaffolding & Equipment. |
Craig Wisbiski - Vice President of Operations
Craig originally joined Form Tech in 1986, shortly after attending Architecture School at the University of Houston. Over his long career at Form Tech, Craig has held various positions in engineering, sales, and operations. Craig served as the company’s Purchasing/Inventory Control Manager for 16 years (2002-17) where he was responsible for vendor relationships and selection, quality of products, maintaining inventory levels at all company locations and directing all inter-company equipment transfers. Craig’s competency in many of Form Tech’s operational processes has made him an invaluable internal resource for many employees throughout the company. In 2017 it was a natural fit for Craig to assume a more extensive role as Director of Fleet & Operations. In 2021 he was promoted to his current position as Vice President of Operations. |
Jim Upton - Vice President of Engineering
Jim joined Form Tech in 1990 as the Branch Manager of the company’s Charlotte, NC operation. Jim’s engineering capabilities coupled with his extensive formwork knowledge and field experience helped the company to expand rapidly in Charlotte and eventually led to the opening of the Raleigh, NC facility in 2004. Jim managed and grew the North Carolina operations of the company until 2017, at which time he took on a broader role as V.P. of Operations. In 2021, he was promoted to his current position as Vice President of Engineering. Prior to joining Form Tech, Jim worked for Lee Construction/National Engineering as a Project Engineer, and later, for Turner Murphy Construction as a Project Manager. He also worked for several years at Symons just before joining Form Tech. Jim began his Civil Engineering studies at Central Piedmont Community College in Charlotte, where he qualified as a Phi Theta Kappa National Honor Society member. He subsequently earned his degree in Civil Engineering Technology in 1981 from the University of North Carolina Charlotte. |
Frank Smiser - Vice President of Sales
Frank joined Form Tech in 1998 as Branch Manager of the company’s Grand Rapids, MI operation. For nearly 19 years, Frank managed several regions across Michigan and Ohio for the company. In 2016, he assumed the role of Major Projects & Key Account Manager, where he was directly responsible for Form Tech’s largest and/or most complex formwork requirements. Frank’s technical expertise, grasp of overall project requirements, and years of field service experience has made him one of Form Tech’s top resources and a natural fit in his current role as V.P. of sales. Frank began his career in 1984 with Greenrose Supply in Louisville, KY where he worked in an outside sales role for 6 years. He joined Symons in 1989 where he worked for 10 years, holding the positions of District Sales Manager, Steel Form Specialist, and finally as Dealer Sales Manager, just prior to joining Form Tech. Frank attended the University of Louisville, and earned a Bachelor’s degree in Business in 1988. |
Lucy Klohs - Chief Financial Officer
Lucy joined the Form Tech team in 2017 as Chief Financial Officer. An entrepreneur, Lucy started her first consulting practice in 2001, earning her reputation by helping business owners position their operations for financial success. She has held key leadership positions in Finance and Accounting in private industry, including Construction Materials and Consumer Goods. Lucy holds dual Bachelor’s degrees from the University of Michigan. |
Guy Williams - President & CEO
Guy has over 10 years of leadership experience in the equipment rental industry. Previous engagements include serving as the District Manager of United Rentals Tool Solutions Division and formerly as President of LEASCO Equipment Services. As the District Manager of United’s Tool Solutions Division, Guy had full oversight and accountability for a $50M+ business unit with over 100 employees across thirteen locations spanning a wide geographic region that stretched from Kansas to New England. Prior to United Rentals, Guy was the President of LEASCO Equipment Services in Marietta, Ohio. LEASCO was focused on industrial tool rentals for the power generation, petroleum, chemical and pulp and paper industries. Under Guy’s leadership, LEASCO doubled in size through organic growth initiatives, leading to the successful sale of the business to United Rentals in 2009. Guy holds a Bachelor of Arts degree from Ohio Wesleyan University and earned his M.B.A. from the University of Dayton. |
